Email Info@cefofcollier.com to request a paper form.  Please tell us your child's school as the form is school specific.

 

Turn in the completed form to your school office or your Good News Club leader.

Good News Clubs are not an official CCPS School District activity, but we are a contracted volunteer "After School Program" led by pre-approved and pre-screened volunteers of Child Evangelism Fellowship of Collier County and local church partners with CCPS and adhering to all governing Child Protection & Safety policies.
